Appreciate it could be a phone issue but I can't seem to figure out how to get an incoming ringtone through the car speakers. Phone is a Pixel 6.

If I have phone on silent or vibrate I obviously get no ringtone from the phone but I'd still expect a ringtone through the car speakers.
If the phone is not on vibrate or silent I get the usual ringing through the phone speakers but again nothing through the car speakers.
Not a massive issue as it silences the radio when I get an incoming call so it's still noticeable but I'd expect an option to get an incoming ringing sound through the Infotainment system.

Does anyone have any ideas?

Have you tried adjusting volume whilst it's silently ringing? Not sure if that works in this case, but alerts like that often tend to have a different volume level to other things that is often only easily adjusted whilst it's happening.

Thanks. Yes, that adjusts the "Audio" volume but seemingly has no effect on the (lack of) incoming ringtone tone.

Update: works fine with incoming call ringtone connected to iPhone.
Tried a different Android phone and got same issue so it's Android specific whatever the issue is.

Edit: Getting there with investigations, it seems my Pixel 2 watch is the culprit. Curiously the setting in the Watch app that I expected to resolve this (Toggling off the setting under Manage phone notifications> Mute phone calls : while wearing your watch calls don't vibrate or ring through phone) didn't change behaviour however if I turn the watch to Airplane mode I can get the incoming ringtone through the car speakers. Notably this only works if the phone is not on silent or vibrate only mode. Could be behaviour by design I suppose but it's different on iPhone - a connected iPhone seems to ring through the car speakers irrespective of whether the phone is on silent or vibrate.

Wifey has had many issues with her watch and car bluetooth, across multiple cars. It seems that Android has no way to set a priority. The phone is connected to both your watch and the car, both of which can act as "handsfree", and theres no way to force it to pick one over the other. Sometimes it picked the watch, sometimes it picked the car.

Android Auto helps make it more reliable, over just using bluetooth.

I also found my Android phone, when set to DND, would completely reject calls when driving, rather than just silently ringing. I had to create a macro which disables DND when it detects the cars bluetooth connection.
